- This website doesn’t use analytics, advertising or tracking cookies.
- The embedded Google Map may set its own cookies.
- Information you send through our forms comes to our office so we can respond to you.
- Your child’s dental and health records are protected under HIPAA and handled by our office, not this website.
Information you give us
When you request an appointment, refer a friend or send a doctor referral, the form collects the details you type in (such as names, a child’s age, email address and phone number) and sends them to our office at info@sammamishpediatricdentistry.com. We use it only to respond to your request and schedule care. Please don’t include detailed medical information in these forms; we’ll collect that securely at your visit.
Third-party services
Some links and features on this site are provided by other companies with their own privacy policies:
- Patient Health History Form and online scheduling open on patientviewer.com, a service our office uses for secure patient forms and appointments.
- Google Maps shows our location on the home and contact pages.
- Google Fonts provides the typefaces on this site. Your browser requests them from Google, which receives your IP address; Google Fonts does not set cookies.
Cookies
This site itself doesn’t set cookies. The embedded Google Map may set cookies subject to Google’s Privacy Policy. You can block or delete cookies in your browser settings; the rest of the site works the same without them.
Health information
Protected health information about our patients is handled according to the Health Insurance Portability and Accountability Act (HIPAA). Ask our front desk for a copy of our Notice of Privacy Practices. Washington residents may also have rights regarding consumer health data under Washington’s My Health My Data Act; contact us with any requests.
Children
This site is intended for parents and guardians. We don’t knowingly collect information online directly from children.
